Pagosa Springs Medical Center has an attractive facility in a scenic town. PSMC is an independent, patient-centered facility led by a physician, CEO Rhonda P. Webb, MD. PSMC operates a critical access hospital, outpatient clinic, and EMS/ambulance for all of Archuleta County. PSMC is licensed for 11 inpatient beds and is a level IV trauma center. PSMC offers a range of specialty services including: orthopedics, general surgery, pulmonology, neurology, cardiology, oncology, infusion and pain management. PSMC has on-site diagnostic imaging including MRI, CT, mammography, and ultrasound together with an on-site radiologist and access to Colorado’s largest team of tele-radiologists. PSMC offers a 24/7 emergency department, EMS/ambulance, physical therapy, respiratory therapy and lab. Eastern Colorado Health Care System (ECHCS) is actively recruiting physicians trained in Family or Internal Medicine to work with a dynamic, collaborative, and engaged interdisciplinary team in delivering high-quality and efficient health care in a Primary Care setting.

Our Primary Care Provider functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model). The PACT team model consists of an MD (or PA/NP), RN, LPN, and Medical Support Assistant. Providing leadership to the team including shared delegation of appropriate care and care processes to appropriate team members. You will be assigned to provide comprehensive, collaborative same-day and continuity care to an assigned panel of Veterans via face-to-face or telehealth modalities utilizing all available tools, such as registries, to enable effective population health management. PACT Providers are expected to demonstrate excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members, and staff.

The Colorado Coalition for the Homeless is looking for a Physician to join a team of clinicians who provide primary health care to individuals and families at clinical sites under the Colorado Coalition for the Homeless – Stout Street Health Center.

Clinical sites include: The Stout Street Health Center (SSHC), satellite clinical sites, the Mobile Clinic, the Street Medicine Program or the Recuperative Center. This position will have a focus on helping build the new Recuperative Center which is situated in downtown Denver. The physician will work collaboratively with the SSHC multidisciplinary integrated health team.
